How to register

New Patients

We have an open list and welcome requests for registration from patients living in or moving to the health Centre area. To understand our boundary, please use this link

Alternatively, you can collect a registration pack from the surgery.

We will require a completed form for every member of your household who wishes to register.

Although providing ID is not required, this does help us find your medical records from another surgery and check you live within the health centre boundary. We need to register you before you can see a clinician or ask for a prescription.

What will happen once I am registered? You will have a named, accountable doctor who is responsible for coordinating your care. You can still talk to or make appointments to see any of our doctors or nurses, not just your named GP.

We recommend that new patients undertake a health check with a practice nurse – we can arrange this straight away for you.

Temporary Patients

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient please contact the local practice you wish to use. For example, if you are studying at university and were previously registered with us, you are also seen as a temporary resident. If you are not registered with Bodmin Road Health Centre but are a temporary resident within the practice boundary you will have to complete a temporary services form to register as a temporary patient.
